Dr. Chelsea R. Evans is a native of Whitley City, Kentucky where she attended McCreary Central High School and graduated as the 2007 Valedictorian. She then achieved her undergraduate degree in 2011 at Eastern Kentucky University where she graduated magna cum laude with a Bachelor of Science in Biology. She went on to attend the University of Alabama at Birmingham School of Optometry and received her Doctorate of Optometry in 2015. While in graduate school she completed internships in primary care, ocular disease, contact lenses, pediatrics, vision training, optical, geriatrics and community eye care.
As a Certified Optometrist, she provides examinations for routine glasses and/or contacts, pediatrics, diabetics, cataract surgery co-management, low vision, and ocular diseases, infections and injuries. She also performs examinations for veterans.
Dr. Evans is an active member of the American Optometric Association, the Florida Optometric Association, West Florida Optometric Association, the University of Alabama at Birmingham Alumni Association, and the Beta Sigma Kappa International Optometric Honor Society. She has licenses in the states of Kentucky, Tennessee, and Florida.
She has been married to her husband, Dr. Robby Evans, O.D., since 2015 and they have a daughter, Ellie. When she is not working or enjoying time with family she adores golfing, gardening, visiting the beach, cooking and crafting. She also has a special interest in volunteering vision screenings for Special Olympics participants.
Dr. Jane Purdy grew up in Missoula, Montana where she attended Sentinel High School graduating valedictorian in 2004. She then received a Bachelor’s of Science in Biology degree from Seattle University in Washington state where she was granted an athletic scholarship to play collegiate softball. Dr. Purdy went on to graduate with distinction from Pacific University College of Optometry in Forest Grove, Oregon, with a Doctorate of Optometry degree in 2013. While in optometry school, she was awarded a Health Professions Scholarship and commissioned into the United States Air Force. Upon graduating, Dr. Purdy served as a Captain in the Air Force providing comprehensive eye care for the 1st Special Operations Medical Group Aeromedical Squadron at Hurlburt Field. During her Air Force career, she earned Company Grade Officer of the Year two years in a row and Provider of the Year as well as the Air Force Special Operations Command Biomedical Science Clinician of the Year. After separating from the Air Force with an honorable discharge, she joined Dr. Batson’s team at Okaloosa Eye Care and Shalimar Eye Care. Dr. Purdy will continue serving in the Air Force Reserves stationed at Duke Field, Florida as an optometrist for the unit.
Dr. Purdy is active in the community continuing her third year as a Guardian Ad Litem for the state of Florida advocating for children removed from their homes and placed in foster care. She is active outdoors enjoying paddle boarding and fishing.
Dr. Purdy is excited to provide a full scope of optometric care including pediatrics, management, and treatment of ocular diseases, low vision, ocular trauma, postoperative care for refractive and cataract surgery, contact lens fittings and glasses prescriptions. She also performs examinations for veterans
